The fifth edition of this classic text is an encyclopedia of useful information. There are three new section editors and much has been added about minimally invasive surgery since the last edition. Most of the chapters are current, very well written, and provide a comprehensive review of the basis of the management of gastrointestinal tract diseases. A number of chapters address topics that often are overlooked or are only briefly reviewed in surgical textbooks.
One criticism of the text is the overlap of information, particularly description of operative techniques, in a number of chapters. It is understandable that most surgeons would find it difficult to compose a book chapter without including operative management, however, it can be difficult to find where operative descriptions are located. In some chapters, the presentation of extensive historical information and descriptions of outmoded operations can overwhelm the reader who desires to use this text as an authoritative source for more modern management. There are a few gaps in content (prosthetic mesh for the repair of hernias) and variable emphasis on evolving techniques (PET scanning, applications of molecular biology to treatment). Some operations are described in extensive detail that is helpful to less experienced surgeons while others are too sketchy in their descriptions and leave the reader wanting for another reference.
A new section on “Incisions” has been added which is unique and useful. One suggestion for a future edition is that the extensive anatomic illustrations be modified to better reflect the relationship of anatomic structures to commonly performed incisions.
This compendium is a comprehensive and useful reference that most surgeons, residents, and interested medical students will want to consult regularly. This text is a “must have” for departmental and institutional reference libraries; however its price may make it difficult to include in one’s private collection.
